1. We know what share of last year’s leavers completed an exit questionnaire or interview.

2. Each month, someone checks the number of leavers invited against the number who actually left.

3. The exit questionnaire or interview is run by someone outside the leaver’s management line.

4. Exit results are analysed by someone with no stake in the teams being described.

5. Leavers are invited at least three weeks before their last day, where their notice allows.

6. Leavers who have not responded get at least one reminder before they leave.

7. The invitation explains, in writing, exactly who will see the answers.

8. No exit results are reported for a group below a set minimum size.

9. Exit results are reported at least once a quarter.

10. Reports separate leavers who were dissatisfied from those leaving for life reasons.

11. A named senior person owns the exit results.

12. At least one change made in the last year can be traced to what leavers said.

Yes scores 2, partly 1, no 0, for a total out of 24. 20–24 is in good shape, 12–19 working with gaps, 0–11 not yet reliable. If any area scores zero, the result is capped at “working, with gaps”, however high the total: one missing area is enough to undermine the evidence. “Where to focus first” is the area with the lowest share of its possible marks; if two areas tie, it is the one listed first below. Any other area scoring under half marks is listed too.
